LUNCH ORDER FORM

Please fill out this Lunch Order Form for EACH of your children EACH week.

For days your children are bringing lunch from home (not having school lunch),  please select "No Lunch Order".

This form will be updated with the new weekly menu and “turned on” every Friday at Noon and “turned off” every Tuesday at Noon.

Lunches are paid for by tokens. Tokens can be purchased by completing a

 Token Order Form

Send the completed token form to the main office.  Your order will be processed and tokens/tickets will be sent home with your student. The student’s FACTS account will be charged. 

Please be sure to send your student to school with a token/ticket each day that they have ordered lunch.  If a student forgets to bring their token/ticket to school, they will still receive their pre-ordered lunch, and the students’ FACTS  will be charged the cost of the lunch.
