2024-2025 After-Care Registration instructions are below. Please enroll early, availability is on a first-come, first-serve basis. The program capacity is based on staff to student ratio, once at capacity a waitlist will be available.

Registration

• Please complete (1) 2024-2025 SMLS Before & After Care Online Registration Form  per student attending, once submitted you will receive an email confirmation
• Complete the SMLS Emergency Medical & Contact Form below, and provide a copy to our Health Office at [email protected] or a hard copy Att: SMLS Health Office

• FACTS Tuition Management Program allows fees to easily be applied monthly Questions regarding FACTS can be referred to Mary Thomas at [email protected]
• Registration fees and enrollment forms must be completed prior to the first day of attendance
